STEP 2: DOWNLOAD AND SUBMIT SUPPORTING DOCUMENTS
All participants must submit the Emergency Medical Information and Consent Form with payment or during registration at Cheeseman Farm.
All participants will sign the Code of Conduct at the YAC, but you are welcome to view it HERE before arriving.
You can print EYD information/registration documents for your convenience.
STEP 3: PAYMENT
YOUTH RATE (17 years old & under): $70.00
ADULT RATE (18 years old & over): $80.00
(Includes: room, meal, T-shirts, and various activities)

Registration cutoff date is June 7, 2018.
*Note: Eparchial Youth Committee can still accept the late registration form, but availability of t-shirts and workshop materials may not be guaranteed.
